Your first appointment is about getting the full picture: your health history, current medications, recent lab work, and how diabetes (or a prediabetes diagnosis) has been affecting your daily life. Plan for a longer first visit so we have time to listen before we plan.
What to Bring
- A photo ID and insurance card
- A list of current medications and dosages, including insulin or oral diabetes medication
- Recent lab results or a referral form from your primary care provider, if available
- Your home blood glucose log or meter, if you track readings
- A list of questions — no question about living with diabetes is too small
